I tested the jotform. I received the submission form however the email and phone number I used did not receive any message confirming their reservations.

As far as I understand you don't receive emails.
Your email address, you4lia@gmail.com is NOT in our bounce list.
I've also checked your Mail Logs. It seems [2015-12-16 02:09:21] Submission #326058961968159599 Form #53418302423446 SENT to you4lia@gmail.com from noreply@jotform.com via AMAZONSES

Could you please check your spam, trash and bulk folders? If you find them there, please mark them as not spam in order to not go into the folder any more.
Lastly, you are currently using a custom sender email address through verification method. If this is not working to some of your forms, I would suggest using our SMTP method(s):
