We are currently in the market for a new form builder tool. We are currently using Drupal Webform. Two functionalities that we currently have are being able to capture the URL of the page the form resides on and being able to capture the Ad campaign that directed the user to the page.
Is this something you've done before? Please, by all means, contact me by phone if you need a better explanation.

By adding that widget to your form, even if the same form is embedded on multiple pages or even multiple websites, the widget will show you on what exact URL was the form filled and submitted on.

When it comes to your Adsource ID, I believe it would be possible, but I'm not quite sure how it is that you usually pass it on or collect it.
- If it is in the form of an URL parameter, you can create a hidden field on your JotForm form that has the same name as the URL parameter, and the form field would be automatically filled with the value of the URL parameter. This is possible because of the prepopulation of form fields through URL parameters.
