Unfortunately, it is not possible to import directly a word document form to JotForm.
However, these are the following import methods that you can use:
If your form is created in spreadsheet, you can use this app: http://apps.jotform.com/app/spread2jot
If you have a word document, you can still import it by first converting the word document to an HTML file, you can follow this guide: http://www.wikihow.com/Convert-a-Word-Document-to-HTML. After that, upload the HTML file on a web hosting site or a server so that it can act as a live website.
