Can one JotForm account have forms on multiple websites? If so, is submitted data from different forms kept in separate databases?

You can create a form and share it via email or embed it in a website, and still will be the same form.

If you are looking to embed it in a website, we suggest to use the IFrame method : http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
You will receive the submissions in your form, even if you have, for example, your form embedded in 10 websites, you will receive all the data in the same page, take a look to this guide in order to know how to view your submissions : http://www.jotform.com/help/269-How-to-view-Submissions
