Can I create an interactive survey?

I am working on a project for a client. She's interested in creating a battery of questions with closed ended questions. The questions and responses for each category will calculate a score at the end of the survey that provides the next action steps based on the score.  She would love to have the final report to provide a chart that shows the score from each set of questions.  

Has anyone done anything like this using JotForm?  Is it even possible?
