I need the same, am using the example above currently however it is misleading to the customer since it has the per month on it. "USD for each month". along with the word Donation to describe it. If we could change those two items this would be perfect, or at least let us have the option for those entries.

The steps mentioned by my colleague in this link, is for subscriptions with custom amount. Please check the following guides:

This guide helps you setup your order form: http://www.jotform.com/help/165-Setting-Up-Your-First-Order-Form

Currently, there are three options that you can select:

    - Sell Products with fixed values

    - Sell Subscriptions

    - Custom Amount/Collect Donation

The one mentioned by my colleague is for subscriptions or recurring payments, that is why there is a label on it that says "USD per month". You can change the subscription period to "Daily", "Weekly", "Bi-Weekly", "Monthly", and so on...

To learn more about recurring payments on payment integration, please follow this guide: http://www.jotform.com/help/166-How-to-setup-a-recurring-payment-option. However, apologies if the "Variable Price" option is not mentioned in the guide, it is quite a new feature on that part of the payment wizard. But we will surely update the guide.

Do let us know if I misunderstood your concern.

I have used the Variable price option and changed the name of the
    subscription field.
Selected monthly as subscription period. The part that needs not to show
is the USD for each month. In my case this is a one time purchase for
services rendered that has no set price, however it needs, in my case to
be on the same payment form along with the subscriptions. In other words
a person can buy a 12 month subscription for x amount of dollars that is
a set price and obtain consulting services for a one time payment that
    is variable.
I guess I could add a page to my website for those transactions and use
the variable price, or look into one of the other price set ups that I
could enter the amount each time.
Be a lot simpler if the "for each month" part was left off the variable
choice that follows the dollar entry amount. At present in the name
change I added One Time Charge so the customer doesn't think he is going
to be billed for this each month.
Below is how my form looks
    My Products *
One Year Subscription ( $ 200.00 USD for each year .)
Two Year Subscription ( $ 300.00 USD for each two years .)
Monthly Subscription ( $ 25.00 USD for each month .)
Misc Services One Time Charge ( USD FOR EACH MONTH) the red is the
    problem

Please be noted that user will be charged as per the Subscription Period selected in the wizard since it is a subscription payment, and hence changing the label would not be helpful. I think it is required to create different forms for subscriptions and Custom payment. However, you can embed the other form with custom payment on your subscription form by following the guide below :

    http://www.jotform.com/help/182-How-to-Create-a-Basic-Multiple-Payment-form

Let me see if I can get back to you with a demo to show how it works. Please allow me some time to work on this.

    Thank you!

We will answer that question on this thread http://www.jotform.com/answers/727781 to avoid breaking the main topic of this thread. We'll be on it shortly. Thanks!
