Logos not showing up properly

All of my forms are not showing the logos in the header properly. This has been happening the last few days. Any idea what is happening? Thank you!

You may have accidentally deleted the logo. It doesn't exist in the list of the images that you have previously uploaded: 

As you could see, the URL is under your name but it's not available. You may consider re-uploading the image to fix it.

ahah! I didn't realize that deleting it from the list of options would delete it from the form. Thank you for your help!

Yes, that is saved under your name so deleting it from one form will also delete it in the other forms. It's not per form, but not unless you will upload it again to other forms instead of just selecting it from the list.
