We are using our "Quote Request Form" on our website www.buttonstar.com. We have been using it successfully for a long time. All of the sudden it does not work. You can fill the form out but when you press "submit" it says "please wait" on the button and never submits. I have put the direct link to the web page that has the form below. I have also submitted a screen shot below.
Please let me know if you need anything else so we can get this fixed asap. Thank you.

However, I have tried using the direct link to your form and I'm able to submit it.
May we know which embed option are you using please ?
I checked your website and seems like your are using the Source Code option.
I would suggest you to try using the IFrame method, refer to this guide in order to know how to do it : http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

I am new to this...Andy is not with us anymore. I am trying the IFrame method now.
So, I got the IFrame to work but it is wider than it was previously and overlaps other links. Is there a way to keep it the same size?

To solve your issue, paste the code provided for the wizard when getting the IFrame code in a text editor, and change the height value to 1400px, as the image below :
Now, go to Designer and paste the next CSS code :
You will find the Designer on the top toolbar, click on it and then click on the CSS tab, there paste the code.
This will allow your form to be displayed properly.

Okay, I did all that and the form got jumbled up in the middle. So, I took the CSS code out. Since it looks better without it. Now the "Item Number" is pushed too far over to the left.

That's fine but I need this first issue fixed. The quote form is still too wide. Can you help me with that first?
