How can I delete the image icon at the very top?

    You can delete it by editing the form and click on the "gear" icon.

    Hi,

    The sample cover is in the form designer. You just need to click "Clear" button from the Form Cover tab:

    This will remove the sample logo from your form.  Here is my test copy of your form with the cover cleared:
