    When I go designer tab my form is resetting its format.

    Did you import the original form from any of our templates? It seems that there is an issue with CSS code of the form. I've cloned your form my account and was able to replicate the issue. However, applying a new theme has fixed the issue and opening the form in builder/designer displays the same. Please try applying a new Theme to your form by clicking on Themes icon to see if that helps.
