How to have a Multiple Taking Photo Form

Asked by ajmesalic on December 17, 2015 at 04:48 PM

Hi, I want to set up a form that is way beyond what I've done before. I started it and it's called Job Intake. Could you insert some field to show me how to do the following?

Take customer email address, name, physical address. Enter number of items to photograph. Take photos of several items, with a line of information text for each. Have a checkbox for each item that we can use to select a service option. Submit all that and have it issue a receipt to a customer with a copy of all the photos and information that was taken.

If you can do that then I will absolutely say you are rock stars.

This is very much possible to achieve using the Form Builder.

The only main limited part is the Take Photo widget.

Since Take Photo will only work after each submission, it will be practical to use only one Take Photo widget on the form instead of having multiple of them on the form to take photo of mutlple items.

To take photo of multiple items, just use the form, take photo, and then submit repeatedly. There will be option upon submission if the user will still want to take another photo or not.

To get a receipt/copy to the customer, add an Autoresponder Email to the form as well.

I will create a demo form and will get back to you shortly.

You can clone  my test form so you can review it on your Form Builder.

You can watch the video below on how I created it and the idea I was suggesting.

Hope this help. Let us know how we can be of further assistance

That is really intriguing. So a couple of questions:
    1. Can the information on the form be made persistent, so we don’t have to re-enter it on a smartphone every time?
    2. Is there a way to attach multiple files from the photo library at one time? I know you said it wouldn’t snap multiple photos.
Wow! Thanks for your great help.

    #1 Yes.

    Test this new form https://form.jotform.com/53508637458970

    Here's how it works...

I simply prepopulate the redirect URL of the form using parameter value.

    example:

    https://form.jotform.com/53508637458970?fullName[first]={fullName:first}&fullName[last]={fullName:last}&email={email}&address[addr_line1]={address:line1}&address[addr_line2]={address:line2}&address[city]={address:city}&address[state]={address:state}&address[postal]={address:postal}&address[country]={address:country}

#2 You can add multiple Take Photo widget and use each to take multiple photo at once on the same submission. I was only suggesting using 1 only for convenience and simplicity. But you do can add multiple Take Photo widget on the same form.

Let us know how we can be of further assistance.

Jonathan, what you did is really brilliant. There’s just one more thing I wish for to make the form perfect. Is there a way to make the number of photo upload modules conditional based on an input? Like question 1 would be how many items are being documented. Then if we say 7 then there are 7 opportunities shown to allow the upload of 7 photos. Like an accordion. The form has formID=53511624042141
I’d also like to know if it’s possible to dynamically put an entered email like {ProSellerEmail} in the send-from and reply-to fields of the email notification. In fact there’s a second notification that I would like to be sent TO that email address as well. All of these appear to be disallowed but we need that functionality.

    Thank you,

    Hi A.J.

Yes this is possible as well. You can review this user guide.

I will create a demo form again that have this applied. I'll get back to you shortly.

Since its about a different thing, we will resolve it separately here http://www.jotform.com/answers/728962

Please check this new test form https://form.jotform.com/53516697303964

I have added conditional statement like this

This was done base on the method described on the user guide: http://www.jotform.com/help/297-Show-or-Hide-multiple-fields-at-once-using-form-collapse-tool

We will address the other feature here.

Let us know if you need further assistance on the the dynamic show/hide conditions.

    Thanks.
