Cannot add fields to Aweber integration

    I set up integration with Aweber through Jotform to test before I use it. The integration did take but only 2 fields (name & email) out of 5 showed up. On the Aweber integration app, there is a "add a field" button but depressing it does nothing. I tried it with 3 different browsers (IE, FF, Chrome). There is a drop down menu with the other fields to select from, but I need ALL 5 fields, not just 2. I have all 5 fields in both Aweber & Jotform. Before I pay for the yearly service, I need to know is there a way to get the add a field button to work, or do I have to use a different form builder?
