I would like to customize the subject line of the email notification to be the name of the person who filled out the form. Is this something your forms can do?Also, the form needs to be able to change depending on certain fields. For instance, the first question would say "what is the purpose of your call" -- if the option 'billing' was selected, the remaining questions would be specific to billing. If the option 'RX Refill' was selected, the questions would be specific to that. Is this an option?

Inside of email Notification settings you can set Subject of your email Notification. You can also add value of Full Name field. So if full name field is filled vales that are entered inside of that field will be shown as Subject of your Email Notification.
