Letter not keeping format or photo

Hello:

I created a form that will send a letter to the recipient.  The letter the recipient receives does not keep the form and the logo does not show up at the top of the page.

Please let me know what Ineed to do to keep the form of the letter and should I add the logo.

I took a look at your form Priscilla and I only saw the email notifier, without any setup with logos.

Based on what you have mentioned, I believe that you are actually looking to send an email autoresponder to the people that fill out the form with the logo of your company. If that is true, you can accomplish that by adding the email autoresponder as shown how here: Creating a Form Autoresponder
