As mentioned on the previous thread, I took a look at the same form as before: http://form.jotformpro.com/form/52844963978981, but the email at this time only shows: "undefined" as the email content.
The email setup is likely to cause you issues at one point or the other since both the FROM and TO headers contain the same email address, so I would suggest to either send the email from email@example.com or to send it over the SMTP server as you had it before.
To fix this current email, you should either manually add the fields, or recreate it by removing this one and adding a new one in its place.
I would suggest removal of current and creating new one, but both ways should work just the same.
