application not fillable

all the boxes are not fillable when i put the link into a new tab

try entering the dogs name, breed, anything...

you can put the cursor at the beginning of the field and type, but you can not see the letters

you might be able to highlight afterwards and see the printing, but not as you are printing

i have gone in and made sure the text is set to black

I have tested your form and text that I entered inside of text box fields is visible. However I noticed that check box option is white so it is not visible. 

To fix this inside of you Form Designer click on any check box option. Radio button & Check box styles tab will appear. Select color dropdown option and change color to e.g. #555555. Don't forget to save changes.
