    Unfortunately, you cannot import the word document directly.

    But this can be done by following the steps below:

    1. Convert your word document to an HTML file.

    2. Upload and host your HTML file to a web server.

    3. Get the web link of the HTML file and import it to JotForm by following this guide: http://www.jotform.com/help/28-How-to-Clone-an-Embedded-Form-from-a-Webpage 

     

    If your word document can be converted to a spreadsheet, then you can use the app Spread2Jot to convert the spreadsheet to a JotForm form. However, please note that importing a document or HTML file won't be able to copy the whole form's design and layout 100%. Because they are created on different platforms.
