    I have cloned your form into my account for testing, and it seems to show correctly. Based on your first screenshot, I have noticed that it is showing several hidden fields, which are consequently invisible on the live form.

    Your fields for Agreement 1, 2, and 3 are all set to be hidden in their properties, which means that not even conditional logic can make them visible again. If you want to make these fields visible, but conditionally hidden, please click on the gear-shaped icon of each of those fields and click on Show Field:

    When a field is set to hidden in there, the field will remain invisible no matter what, even if there are conditions set to show them. Please let us know if this resolves the problem you were experiencing.
