my jot form does not include the email address of the sender

I see nothing wrong with your form email settings. However, your email field is not required so someone can just skip it. You may consider making it required so to let users fill it before submitting the form.
If you would like to get the form email and have the submitter's email address as the sender email, you will have to setup Mandrill's SMTP to your forms. Here's how:
