When we switched our site to WordPress we used the code you provided and everything was working fine.

Now mysteriously the forms have disappeared. We have not installed any new plugins. We are using Wordpress 4.4 and Thesis Theme.

Here are a few of the pages that have forms:

    http://darksky.org/idsp/become-a-dark-sky-place/dark-sky-park-inquiry/

    http://darksky.org/idsp/become-a-dark-sky-place/dark-sky-reserve-inquiry/

If I try I direct link or view the form directly in JotForm I get the error "

    Form over quota
This form has exceeded its allocated quota.

Please let me know how to fix this issue.

    Hi,

I see that you are using a Free account in Jotform. Free accounts only have 100 MB of available storage. Your account has exceeded the 100 MB limit and is currently at 781 MB, which is why you are getting the "This form has exceeded its allocated quota." error. You may delete your submissions with uploads so you can regain your space back, although keep in mind that once they are deleted, you can not get them back. If you prefer to keep the submissions, you can also upgrade your account to gain more available space. Just visit the pricing page and check out this article if you need help with upgrading: How to upgrade my account

Do inform us if you need further assistance,
    Regards.
