A couple of day ago you tested my form and I did testing and it seemed to be working properly again and now today, someone used it to upload a video to my dropbox and while the form works and the video was uploaded to my dropbox account, I did not receive any email notification. Just happened to see the new video file in my dropbox account. I do see the submissin in my account at jotform so all is not lost, I can see who submitted the video but it is very frustrating that eveytime I report this, then when you test and then I test it works but then it fails when a user make a submission.
Please let me know if you have any ideas on how to resolve this.

Have you checked your Spam folder for the missing notifications? If you can't find them there, they might have been blocked by your mail provider due to a DomainKeys issue.
To resolve this, kindly change your notification's Sender Name and Email to JotForm and email@example.com, respectively:
After doing so, do some tests then keep us posted on your results.

I changed the notification sender name and email per your suggestion. Had our IT guys whitelist your IP list on our mail server and then did a test submission. The video uploaded to my dropbox but I DID NOT receive a notification email and I do not even see the test in the submissions list. Gone from bad to worse here. Any other ideas?

Just notice on my upload form after the upload completed, rather than getting the confirmation message there, the pay is showing just the captcha and submit button but the rest of the page is blank.

Definately seems something has gone awry with that form.
I cloned the form and did a test submission and I believe the emails have been issued as we would expect/hope.
Can you confirm that receipt of those emails?
If you have recieved the notification emails, you will need to clone your existing form and re-embed its new URL embed code to your website.

I cloned the form and then did a test submission, the video was uploaded and I received the notifications. For now, it appears the problem has been resolved.
