My form is no longer working. Everyone who tries to fill out the form gets a 502 Bad Gateway screen. Why is this happening? It is very important for the form to be back online. I am paying 19.99 a month for this service and it is not acceptable.

I have inspected form you provided and it is working correctly on my end.
I have also done test submission and when I submitted your form I got redirected to Thank You page.
It was probably temporary issue. If you have embedded your form to website, please provide us URL of your website so that we can test your form there.
Also when I inspected your form I notice that you did not set email Notification. If you want to receive email Notifications when your users submit your form, inside of this guide you can find how to do it: http://www.jotform.com/help/25-Setting-Up-Email-Notifications
