Custom Design Changes Not Showing

Hello :) I'm creating some forms for my website, but I've ran into an issue with the form design changes not showing. I saved the finished form and published, but when I click either preview or embed the code into my site, the form shows only the default form styling. The attached screenshot shows how it should be looking.

Am I missing something simple or is this a bug?
