Not receiving recipent notification

    There's always a invalid date error before the signature and the screen hangs.

    I have tested your form and I was able to submit it. When I submitted your form I got redirected to your thank you page.

    After that I have checked our email log and my submission like all others was sent successfully to email address juzplay.waiver@gmail.com which is set as recipient email address inside of your email Notification settings.

    Also I have checked and your Email address is not oi bounce list.  

    Please check spam, trash and bulk folders and if you find them there please mark them as not spam in order to not go into the folder any more.
