What Randy (a fellow JotForm user like you going by the name grade4pagasa) did on his sample form can be seen here: http://form.jotform.me/form/52735555180457
It is basically a Configurable List widget that has 2 static fields / labels and 2 dropdowns following them.
While this would work for short questions and short selections, I have a feeling that you intend to have a bit too long terms of service for this approach.
My suggestion would be to add 1 Text field to your form with the terms of service text (part 1) followed by a dropdown and any additional field that you need and then continued by another field.
This would allow you and your users to see the form, but also get the same in the PDF and email much easier - since it is the terms of service.
If you plan to have it on your form only, it might be possible through Configurable List, but I would again suggest using the approach above.

Thank you very much for your help.
What Randy did it's exactly what i'm looking for just 2 Dropdown fieldinside + 1 text box (2 char space) and bunch of text as i saw its a Narrative field widget. Those 2 dropdowns are to allow our sellers to select witch company is signing the lease agreement template. we have 5 differents companies and at the end of the form customer has to sign so i can generate the PDF for printing. What i need it a text block with a few varaiables and the rest is simple text.

Once you clone the form you will be able to see what Randy did in the form.
What i need it a text block with a few varaiables and the rest is simple text.
I assume with variables , you are referring to the drop downs and the text box you want to include in the widget, since if you are trying to pass values or fill the fields with prepopulated data you will need to use another guide, but please let us know if you are able to achieve what you want, we will be glad to help you.
