My prepared form to be uploaded is not jpg,jpeg etc
I want to send my survey form to abount 400 responders through e-mail and would like to get their response when they click the Submit button
Tell me what I can easily extract data I would collect from responders

I'm not quite sure if I am understanding your issue correctly or not as your title is different than your details.
Based on your above explanation it seems to me that you are saying you are unable to upload your form from word and pdf and that you want to send an email of your form to roughly 400 people?
Is this for a form that you have that already exists?
I'd suggest starting with the form link or pdf of your jotform (I presume) and add that to your email then send that multiple times to all of your recipients that you want. When they receive it they can then open it and fill it in at which point you can receive it after they submit it. When you receive it that data will be sent to your email account so long as you have an email notification attached to send you notice after its been submitted to you.
Alternatively, you can send it via email through your account but be advised depending on your account plan you can only send so many per hour because our server limits it to prevent abuse.
If you meant something else just let us know.

What I would suggest is creating your form then you can distribute it accordingly to your recipients.
Using the form's direct link after you have created and emailed it your users they can then fill it in and submit your required information back to you. If you have set up your email notification for it then you would also receive an email notice of such as confirmation.
Alternatively, if you don't want to use the form's direct link you could send them a pdf of your form instead but be advised that it may not fully resemble the same form you built via the form link.
Also, please note that you would have to email your form to all of your recipients or share it online on a webpage or blog or something so that they can access it.
If you plan to do emails for this from within your jotform account it's not possible to mass-mail this to all 400 of them at once since that would be considered as spamming and so our system limits this some to prevent abuse. Free Plan accounts can email the form a few times per our through your account but you are limited to a few. This limit is raised some with each higher plan level.
