broken graphic on email notification

getting a broken graphic on email notification for this form. I've tried this link:

as well as this one:

https://files.jotform.com/jufs/criterions/form_files/jotform - banner - e-prescribe.png

Then seem to both work in the setup, but not when they're actually sent.

I cloned your form to test it and the image in the email comes through in the autoresponder:

However, it does not come through in the notification:

It looks like it has something to do with the PDF attachment.  When I removed the attachment from the notification:

The image then showed in the email.

Try removing the PDF attachment and adding it back in as a link in the body of the notification instead:

That should allow for both the PDF to be included and for the image to show.
