Problem started when I added a check box. I have cleard browser memory and shutdown and restarted machine, Still the same problem. HELP!!!Page URL:

I can now see a checkbox on your form, I think it's all fine now? Can you please recheck? Sometimes forms can be little out of date since caches might take some time to be cleared. But, they will always work faster. Form updates should always be instant. When it's not changing instantly on your end please allow some time for the form to update, if it tooks so long, clearing your browsers cache would help.
