I checked your form and I can see you have not any Email notifier nor anyAuto Responder.
This guide will help you to create an email notification : http://www.jotform.com/help/25-Setting-Up-Email-Notifications
And this other will help you to create an autoresponder : http://www.jotform.com/help/26-Creating-a-Form-Autoresponder

I'm afraid that we didn't receive your image, because our forum software can't accept inline images sent through email. You can attach images to your post by opening your support thread directly in the browser:
While typing your answer directly on the support thread, you can attach images by using the toolbar at the top. Please take a look at our guide on how How to include screenshot image in the support forum.
We will be happy to assist you in resolving whatever issue is preventing you from setting up your email notifications. Thank you.

I am sorry but I can not upload the image because pdf and Word are not allowed. Do you have a normal email I can send you the image to?

You can send attachments to our support email address: firstname.lastname@example.org
