Headers and text won't show in the downloaded PDF of submissions

I am unable to show text and headers even though that check box is selected under the submission tab. Is there something wrong with my form that is not letting that information to show in a pdf?

Just a side note, even when I click get PDF none of the text of my contract displays and like stated before I have went to the submission section and made sure the show headers and text box is checked.
