why is my form disabled?

    I see that the account is autosuspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. As I check your account, I notice that your JotForm titled "DTD Credit Card Auth. Form" is collecting credit card information on plain text fields which is not allowed by JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on Phishing.

    It is required to use one of the payment tools available in order to collect payments.

    These payment tools are complying with PCI security standards and safe and secure to receive payments online. I've deleted the guilty fields on your form and activated your account. You may now login to your account and see your forms.
