I see that the account is autosuspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. As I check your account, I notice that your JotForm titled "DTD Credit Card Auth. Form" is collecting credit card information on plain text fields which is not allowed by JotForm.
