How do I create a question that requires a numerical value (1-100) or checking N/A for a list?

    I asking coaches to rate their team members on a scale of 1-100.  In some cases they may not have enough information to provide a rating in which case they could select N/A.  I would like to make this a required field.

    In each case I would include a row for each team member.

    Thank you for contacting us. You have two option to complete this.

    1) You may use the Matrix field. The only downside to this is that all the fields are the same type. You can not have a text box or radio button in a matrix

    2) You can use the CONFIGURABLE LIST WIDGET. For me this would be the best option. As this is more dynamic than the matrix field. This will let you introduce the names of the students, score and if it is applicable.
