Why was the form disabled?

2.  The Credential Renewal 2015 form (https://form.jotform.com/51744581696972) was disabled.  There was not an expiration date entered for this form as far as I can tell.  I tried to pull the revision history to see when that form was last edited.  It is showing movement from 2 months ago, but the disabling of this form was done recently within the last week or so.  Are you able to look up and see when that form was disabled?  I have already enabled it again about 20-30 minutes ago, but we are just trying to figure out when it happened so we can pinpoint if someone made an error on our side.

I also can't find any changes through your account history. If that happens again for some unknown reason, let us know so we can investigate further and submit it to our developers. It's either a glitch or a human error then another case where revision history failed to log the recent changes.
