issue with form on submission - getting an error

    need assistance asap.  we use this form all day long.  thanks in advance.

     

     

    here is the error.  I tried chrome, and ie.

     

      HTTP 500 error That's odd... Microsoft Edge can’t find this page

    This page can’t be displayed, because this site’s server might be under maintenance or there could be a programming error.

    I've tested your form, it is working properly on my side.
