I've checked your webpage on on my android device and the form is displayed without any issue.
I've also tried sending a test submission which worked fine at my end. As I check the form on a virtual device which seems to be working fine.

That is very good news, thank you. It is still not displaying on my Samsung S4. It used to, but for a few days it's not there (on any page). I'm not sure what to think/do. Have you heard of such a thing?

I've also checked on a virtual device of Samsung S4 and I was able to see that the form is displaying properly.
Could you try opening a new private/incognito window in your mobile browser and see if the form is displayed properly? Please refer to the guide below to open private window on your mobile.
