Add a disclaimer on the form?

I was wondering, is there some kind of disclaimer I can put on my form for the e-signature? I want to make sure that when people sign it (whether they are doodling or legitimately signing) that they are agreeing to everything listed on the form. Just didn't know if you guys had a template made up of an e signature disclaimer I could use so people don't try and get out of something.
