Add a disclaimer on the form?

Asked by blakereynoldz on January 06, 2016 at 01:13 PM
I was wondering, is there some kind of disclaimer I can put on my form for the e-signature? I want to make sure that when people sign it (whether they are doodling or legitimately signing) that they are agreeing to everything listed on the form. Just didn't know if you guys had a template made up of an e signature disclaimer I could use so people don't try and get out of something.

Yes, you can easily put a disclaimer on your form.

You can check the following widgets:

    http://widgets.jotform.com/widget/terms__conditions

    http://widgets.jotform.com/widget/short_scrollable_terms

To add a widget, you can follow this guide: http://www.jotform.com/help/252-How-to-Add-a-Widget-to-your-Form

Or use a simple check box field like this one.

Let us know if that helps.
