Troubleshooting - Options not appearing on form for end user

    Hello,

     

    I am having an issue where a few end users are unable to select options in the following form:

    The first instance of issue:

    When the individuals select the year 2015, a list of dates populate which should go all the way down to December 28th. When the users select the December 28th date, the document that is supposed to 'show' is not appearing. 

     

    The second instance of issue,

     

    The individuals are not able to see the year 2016.

    I'm checking your form and cloned it in my account, it is quite long and having problems loading it in my end.

    I checked your form and it seems to work as expected. I clicked the year 2016 and it shows a specific week. I also selected 2015 and selected "December 28" on the week and it shows the image and the text field. Here's a screencast:

     

    I see that the form has 140+ conditions which is causing loading problems. If you are still having problems, please let us know.
