My Form Still Does Not Work. I erased the entire thing. Lost all of my submissions and it's till screwed up out of no where. It was never touched and just started having error problems. This really sucks ass.Page URL:

I checked your form and I can see you have a phone number field which must be confirmed before to submit the form.
I have entered a test form and I could not confirm it. However, your users should confirm the form before to submit the form.
Try doing a new test, if the issue is still there, provide us the steps to replicate it, we will be glad to assist you.

The phone confirmation is not the problem. The phone numbers were confirmed on ove 40+ submissions when the form was working.
Are you not understanding what I'm explaining....?
The form was working perfectly.
I DID NOT CHANGE ANYTHING.
Now it does not work....

Thanks for your reply and apologies for the inconveniences caused.
I went ahead and tested your form but I have no problem submitting it.
I think the problem is most likely on the photo when you don't press it's upload button and proceed to the next fields.
So when you submit the form, the upload field will process the file first before it continue to automatically submit the form after that.
Here's an example:
Note that the upload field will not automatically upload the image after your browse it form your computer until you click the upload button or submit the form.
