The prepopulated form won't load and I'm getting an error

My form is https://form.jotformeu.com/beatsahead/new-site-booking-form-page-2 it is the third in a series and  the information from the second form is

I can't see what is wrong with the code but now the page wont load and I get the below error.

If I take out the name prepoulates

&name[first]={yourName:first}&name[last]={yourName:last}

it works properly,

The page doesn't give me any issues.

The name is not there so I can't see the error. Can you bring it back?

The error is also not related to Jotform. Seems like an error in your website files. Please bring back the original flow and let us know so we can try it again.

the field 'name' must have been linked to something within the theme, changed it to your_name and working perfectly.
