Form just stopped working out of the blue. I didn't alter or change anything at all. It just stopped one day on its own. Talked to somebody who's been emailing me only for them to tell me that it was because they have to confirm their cell number but that is not the reason because over 40+ models confirmed their numbers easily.
I don't understand this. Does anyone know of another service?Page URL:

We do have a widget that would let you ask users to confirm their phone numbers, but you don't have this in your form so I'm not sure how that happens to the particular person.
I have tested your form and was able to submit it without any verification or issues at all. It's probably a user error.
Do you know if they are filling it using a mobile device? Or a PC and what browser? Can you ask them to provide a screenshot of the problem?

I just checked it again, your old form actually have our SMS Verification widget: http://www.jotform.us/form/52599179981173. Your new form does not have this widget and you probably changed it already.
