Expanding / Payment Functionality of a Form desired

I want to build a contest entry payment form similar to this form, attached, that we had custom built previously (no longer can be used). We'd like to build this form in JotForm, is it possible to have a Radio Button selection like seen below, one for 1 entry, one for 5 entries, and one for 30 entries – so when the 5 entries or 30 entries are selected it shows 5 or 30 Guess entry lines? Basically, the form will expand to show 5 or 30 entry lines... (with dropdowns for month/day/hour/minute/second & high temp)

We want the form to automatically calculate the amount due based on the Entry radio button selected ($5 for 1 entry, $20 for 5, $100 for 30 entries).We will likely use Stripe for our payment system (are there any monthly fees associated with using Stripe through your JotForm system?)

Is this type of functionality doable in the JotForm system? If so, do you have any instructions on how I can create a similar form (I'm a web designer), or do you have staff on hand to develop such a form (and if so, what would it cost?)

Like my colleague mentioned this is possible to achieve. 

However you would need to create 30 "Guess entry lines" and then you would need to use Conditional logic to show 1,5 or 30 fields based on selection in radio button field.

Other way would be to use Configurable list widgets. You can set three Configurable list widgets. First will have 1 entry, second 5 and third 30.
