OK. So I used a form that you had already and I changed by information. Then I went to Blogger put in the source code and it worked. Then I tried every blogger I could find, my website, pretty much anything that takes html and nothing worked. I would come up blank with the exception of a wordpress site I have, the words showed up but not the form.

I would recommend using iframe embed code if you are embedding it on blogger or wordpress platform while having that issue. This should not create any problems for as long as the platform supports iframe embed. http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

Can you please provide us the website URL where that occurs so we can inspect it here? It would be 10 times easier to resolve this problem if we can diagnose it in our browser. Thanks! :)
