Not all fields are sent over email

    The form you posted is formatted correctly, but when I test it, the notification email does not include name, phone, or email; it just includes the message text.

    How would I go about resolving that?

    To resolve that, simply recreate the email notification on that form. Delete the existing notification and create it again. This will generate a new email template with all the updated field names.
