Change check boxes to Radio buttons with Y or N selections.

    I have an existing form that the doctors now want to have a Y or N answer beside each item on the patient history portion of the form instead of just a single check box.  Is there an easy way to change the single check box to two "Y" and "N" radio buttons?  

    I'm trying to limit the length of the form, so I'd like to have the "Y" or "No" beside each item.

    You can achieve this by adding Radio button field to your form. Then you need to set options for that field and you can also Shrink your fields to take less space.

    There is no easy way to change check box field to radio button field. You will need to delete check box field and add radio button field.

    Thanks for the response and help.

    We would need “Yes” “No” radio buttons on the same line next to each question as the parent would be answering yes or no to “Skull Fracture” as well as “Concussion” for example.

    To achieve that you would need to add multiple radio button fields and than you would probably need to position all of those fields with CSS so that they are aligned with checkbox options.     I would suggest you to use Matrix table. In Matrix table rows, you can add your Check box options and as columns you can set Yes and No. You would also need to set Matrix table options to be radio button fields. You can find Matrix table inside of Survey Tools tab.

    Is there an easy way to get the Matrix Table to load on to the proper line. Setting up the Matrix Table is easy, but when I try to move it to the proper line, it jumps around and often ends up on the very bottom or top of the form.
