    Form logo is not showing up in the form

    Hello, in the past I created surveys, using you system, and was able to add the image of our logo.  The logo would appear.  Now, my logo does not appear when add to new surveys.  It is a  a box with an X.  When I go to old forms the logo is now a box with X.  What is happening? Thanks you.

    I cannot replicate this problem in the given form. I checked it using a different PC and the logo is showing up there.

    If the issue persists to your end, I would suggest changing the URL protocol from HTTP to HTTPS:
