why my forms were disabled?

I checked your account and I can see you are using your forms to ask form Facebook accounts login data, I have removed the password fields of your forms, since  it's against our Terms of use.

I went ahead and removed all the fields related to login credentials, I have whitelisted your forms and re activated your account, if you are still asking for login credentials your forms will be disabled again and your account will be deleted.
