    Thank you for contacting us.

Form that is embedded on employee only page of your website is created on account mybestfriendservices.

Inside of your embedded form 2016 year is not shown in dropdown menu. However on standalone version of your form year 2016 is shown in dropdown menu.

When I inspected your website I saw that you have embedded your form with full source code. We recommend using of iFrame code. Replacing full source code with iFrame code should resolved mentioned issue. Inside of this guide you can find out how to get iFrame code of your form:

    http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

Or you can edit your full source code and add code for 2016 year. You would need to add this code to each year dropdown field.

    <option value="2016"> 2016 </option>

Hope this will help. Let us know if you need further assistance.
