Account suspended due to phishing suspicious.

Hi I created a simple form and I get FILE NOT FOUND when I try to preview. I also published it so I could email to myself.  The link emailes also said FILE NOT FOUND. What am I doing wrong?

It happens when our anti phishing system detects a form asking for data that is now allowed in JotForm, then the form is disabled and the account suspended, however, I checked your form and I can see it's clear, I have restored your account and you are now able to load your form without any problem.
