Form submissions are send to the wrong email.

    I used a template from another user as the basis of my form.  I edited the email settings and even sent test emails that were all correct.  However, when someone fills out the form and clicks the "submit" button,  the email notification does not go to me, but to the person whose template that I used.  How can I fix this?

    The form is set up to send the submissions to tm****en@g*********
